Services: Transitions
Program Description
Transitions is a primarily a socialization group for individuals with serious and persistent mental illness (SPMI). The group meets once per week and involves a time for checking-in as well as a skills training lesson.

Check-in time gives clients an opportunity to report on mood, medication compliance, sleep pattern, suicidality, and other areas of concern. Transitions facilitators assess client safety, stability and progress during this time.

Skills training lessons in the past have covered topics such as:
How to be more assertive
Stress Management
How to deal with difficult people
Budgeting
Reaching Goals
Promoting Positive Self Talk
How to deal with anger
Who Can Benefit?
Transitions is designed for individuals who struggle with:
Symptom management
Lack of social support
Difficulties in their home environment
Difficulty with problem solving
Obtaining and maintaining support services

Diagnostic categories appropriate for referral include, but are not limited to:
Depressive Disorders
Anxiety Disorders
Psychotic Disorders such as Schizophrenia
Bipolar Disorder
Cognitive Disorders
